The protagonist, a rebellious 19-year-old American girl, finds refuge on a remote island off the coast of Chile after falling into a life of drugs, crime, and prostitution. Reflecting on her new life as an exile, she recounts the persecution she endured from assassins, the police, the FBI, and Interpol in the United States.

As she settles into her new home, Maya befriends unforgettable characters, including a torture survivor and a very special dog. She discovers an important family secret that changes her perspective on the events of her life, and she understands the true meaning of love and loyalty.

According to Allende, the inspiration for writing Maya's Notebook came from real events: the drug problem her husband's children faced. The eldest struggled with heroin for years, in and out of various rehab clinics. His daughter died of an overdose, having even turned to prostitution.

"I'm Maya Vidal, nineteen years old, female, single, without a boyfriend—for lack of opportunity, not because I'm fussy—born in Berkeley, California, with a US passport, temporarily taking refuge on an island in the south of the world. They named me Maya because my Nini is fascinated by India and my parents couldn't think of another name, even though they had nine months to think. In Hindi, maya means "spell, illusion, dream." Nothing to do with my temperament. Átila would suit me better, because where I set foot, no grass grows."
